Printable Drug Test Result Forms
Download a free, fillable drug test result form for every drug test cup we sell. Each form is matched to its cup: the panels, abbreviations and cutoff levels on the form are the ones on that device, so the person reading the test only has to tick a result for each panel.
Every form records the test and donor details, the temperature and specimen validity checks, a result for each panel, the overall result and laboratory follow-up, and collector and donor signatures. Fill it in on screen or print it on US Letter paper.

Before You Use These Forms
- Preliminary results only. Rapid drug tests give qualitative, preliminary results. Confirm non-negative results with a laboratory method according to your program’s procedures.
- Not for DOT testing. These forms are not a Federal Custody and Control Form and must not be used for DOT-regulated testing under 49 CFR Part 40.
- CLIA-waived or Forensic Use Only. Each form states the intended use of its cup. Check the product page and insert for who may use the device.
- Cutoffs are detection thresholds. A cutoff level is not a measure of impairment.
- Follow the product insert. Read times, sample volume and validity color charts come from the insert supplied with your device.
Frequently Asked Questions
Are these drug test result forms free?
Yes. Every result form on this page is a free PDF download. There is no sign-up and no limit on how many copies you print.
Can I fill in the result form on my computer?
Yes. Each form is a fillable PDF. Open it in Adobe Acrobat Reader or most web browsers, type in the test details, tick a result for each panel, then save or print it. You can also print a blank form and fill it in by hand.
Why does each drug test cup have its own form?
Cups test for different drugs at different cutoff levels. Each form lists the exact panels, abbreviations and cutoffs of its cup, so the person reading the test records a result for every strip on that device and nothing else.
Can I use these forms for DOT drug testing?
No. DOT-regulated testing under 49 CFR Part 40 requires the Federal Custody and Control Form and laboratory testing. These forms are records for non-DOT rapid screening programs only.
What does a non-negative result mean on the form?
A non-negative result is a preliminary result. Mark it on the form, send the specimen for laboratory confirmation according to your program’s procedures, and record the laboratory name and the date sent in section 4.
Do 90 mL and 120 mL versions of the same panel need different forms?
Use the form listed for the exact cup you bought. Some 90 mL and 120 mL cups share panels but differ in cutoff levels or adulterant pads, so each product has its own form.
